These forms are required for all registered preschool students. Required paperwork is up to date.

Background Clearances
These forms should be completed by any adult (parent/grandparent/guardian) wanting to attend one of our field trips with a student. These only need to updated every 5 years. You do not need to complete the FBI fingerprinting unless you have not lived in the State of Pennsylvania for more than 10 years.
